At Momentum Health Collective, we are a team of allied health professionals committed to helping people live safe, confident and independent lives. We have been providing high quality allied health care services since 1990. We offer personalised therapy services across the lifespan from early childhood through to older adults in the clinic, at home, in the community and via telehealth.

Tony is a highly experienced physiotherapist with over 35 years of private practice & elite level sports experience.

Peter Moussa

Peter is a University of Sydney–trained physiotherapist who has worked with NRL clubs West Tigers and Cronulla Sharks, the Sydney Kings (NBL), and the Australian Rugby League National Team.

Pamela Yacoub

Graduated from The University of Technology Sydney with a Bachelor of Sport Science and Master of Physiotherapy. Pamela is one of our senior Physiotherapists.

Do you accept plan-managed participants?
Yes. We accept plan-managed NDIS participants across all our services.
Yes. We are NDIS registered and can support NDIA-managed (agency-managed) participants, as well as self-managed participants.
Yes. We offer clinic-based, home and community

To begin services, we typically require:• NDIS number• Plan start and end dates• Funding type (NDIA-managed, plan-managed or self-managed)• Relevant reports or referrals (if available)• Support coordinator details (if applicable)

Yes. We regularly provide NDIS-required reports, including:• Functional Capacity Assessments (FCA)• Progress and outcome reports• Assistive Technology (AT) letters• Home modification reports• Mealtime management plansThese reports support plan reviews, funding requests and goal progression.

How can I provide feedback about your services?

We welcome all feedback — positive or negative. You can:

  • Speak directly with your therapist or support staff

  • Contact our team by phone or email

  • Use the feedback form available on our website

Your feedback helps us improve our services and maintain a safe environment for everyone.

What should I do if I have a complaint?

We encourage you to talk to us first. Most concerns can be resolved quickly by:

  • Speaking with your therapist or support staff, or

  • Contacting our team directly

We take all complaints seriously and aim to resolve them as soon as possible.

Once you raise a concern:

  • We will acknowledge it within 2 business days

  • We will work with you to resolve the issue promptly

  • If needed, we will agree on clear next steps and timeframes together

Yes. You are welcome to bring a:

  • Support person

  • Advocate

  • Family member

  • Nominee

They can assist you at any stage of the process.

If an incident impacts your safety or wellbeing, please contact us immediately so we can take appropriate action.
Some incidents must be reported to the NDIS Quality and Safeguards Commission, and we will do this in line with our legal obligations.
If you feel your concern has not been resolved, or you prefer to speak with someone outside our organisation, you can contact the NDIS Quality and Safeguards Commission directly.
